Who We Are
The healthcare people receive in a hospital or clinic is only a small part of a person’s overall health. Nearly 80% of a person’s health is determined by factors outside of a traditional healthcare setting. That’s why our commitment to advancing health equity goes beyond the walls of our facilities and reaches out into the community.
M Health Fairview's Community Clinical Care team brings free health services to community members in the Twin Cities and across the state. We partner with trusted community organizations to reduce barriers including transportation, cost and language.
Free Health Services
Since 2006, the community clinical care team has been providing a variety of free health services through a nursing-led model, including:
Vaccinations
Vaccination clinics through our Minnesota Immunization Networking Initiative (MINI) provide no cost vaccines in trusted community spaces. Since 2006, Fairview's MINI program has provided over 200,000 free vaccines across Minnesota.
