Patients are responsible for any charges if they are Self-Pay (uninsured or opting out of using insurance coverage), or if charges are not covered by their insurer or they have not yet met their deductible and/or out-of-pocket (OOP). To determine what an insurance company does and does not cover, the patient must call their insurance provider. You are expected to pay your bill within 30 days of receiving your statement in the mail.

In addition to the programs available under the Financial Assistance Policy (Charity Care), uninsured Minnesota and Wisconsin residents will be eligible for the uninsured discount. This discount is based on the Attorney General Collection Standards agreement and is not a charity care program.

A hospital-based clinic refers to a clinic owned and operated by a hospital, though it may not be situated within the main hospital building. Some health insurance plans cover care provided in hospital-based clinics differently compared to standalone (freestanding) clinics. As a result, you may encounter additional facility charges associated with hospital-based clinics, depending on how your insurance handles copayments, coinsurance, and deductibles.
If you have questions regarding your benefits and coverage for services at hospital-based clinics, it's advisable to consult your insurance provider to understand how they handle facility charges in locations designated as "hospital-based outpatient" or "provider-based." These locations are sometimes referred to as "hospital outpatient place of service."

Your bills depend on the clinics you visit and the services you receive. The out-of-pocket costs that you pay may be different depending on whether you receive care or tests in a freestanding or hospital-based clinic.
Because of your own health insurance plan, an outpatient hospital visit may cost more out-of-pocket. It may be through an increased co-pay, co-insurance, or deductible amount. Your insurance company can explain the cost difference between services in a hospital based clinic and those in a freestanding clinic.
