Nominate a Nurse
Nominating a nurse is easy. Patients, families, and colleagues can go to the DAISY Award nomination website, choose "Minnesota," and search by Fairview or Grand Itasca to find the facility. Nominations links are available for our hospitals, clinics, surgery centers, and systemwide services. Internal staff can also nominate nurses who deserve recognition for their work and their impact on their colleagues. Advanced practice nurses are also eligible to be nominated for the DAISY Awards.
On the nomination form, write a story about what the nurse did that made a difference in the patient's experience or how they exemplified our Nursing Professional Practice Model.
How are honorees chosen?
Each site's coordinating committee will choose honorees using selection criteria that aligns with our Professional Practice Model. Award winners will be honored by their leaders at an award presentation and will also receive a DAISY award kit, including a DAISY sculpture, honoree pin and a cinnamon treat (which was Pat’s favorite treat). Award winners will also be featured here on the Nursing Recognition intranet page and may be recognized in newsletters or blog posts.
Patients and families will see information about DAISY on posters and banners in our facilities and in their patient information packets.

Introducing the DAISY Award® for Extraordinary Nurses
Patients and colleagues now have a new way to thank and honor our outstanding nurses. The DAISY Award® for Extraordinary Nurses is an international nurse recognition program that celebrates the compassion and skill nurses bring to patients and families every day. The nurse recognition program is based on what is most meaningful – stories of patient and family gratitude for their extraordinary compassionate care.
The family of J. Patrick Barnes established the DAISY Foundation in 2000. During Pat's hospitalization for an auto-immune disease, his family was deeply touched by the immense clinical skill and the enormous compassion shown to Pat and his family by his nurses. When Pat died, his family felt compelled to say thank you to nurses personally. So, they created DAISY. Today, the DAISY Award proudly honors nurses wherever they practice, in whatever role they serve, and throughout their careers.
