You have the right to an estimate of the charges for care. You also may speak to a financial counselor before you agree to a suggested procedure. There are three different ways to obtain an estimate for the cost of care:

Minnesota statute 62J.812 requires our primary care clinics to post charges and payments for 25 of the most commonly billed procedures, and the average payments or reimbursements received for those services from government and commercial insurance. You can review this information here:
Prices listed do not reflect what you may owe for your care. The amount you may owe out of pocket for your care depends on the type of insurance you have and your insurance’s contracted rate with our health system.
If you would like to get an accurate estimate of the payment rate M Health Fairview will receive related to your care, and/or an estimate of what you may need to pay out of pocket, please contact your insurance company.
A hospital-based clinic refers to a clinic owned and operated by a hospital, though it may not be situated within the main hospital building. Some health insurance plans cover care provided in hospital-based clinics differently compared to standalone (freestanding) clinics. As a result, you may encounter additional facility charges associated with hospital-based clinics, depending on how your insurance handles copayments, coinsurance, and deductibles.
If you have questions regarding your benefits and coverage for services at hospital-based clinics, it's advisable to consult your insurance provider to understand how they handle facility charges in locations designated as "hospital-based outpatient" or "provider-based." These locations are sometimes referred to as "hospital outpatient place of service."

Patients who prefer paper statements can easily switch their default delivery option back to U.S. mail within MyChart or the M Health Fairview mobile app. Follow these instructions:

If your child is under 18 and you are responsible for their account, you will receive the statement on MyChart. Anyone 18 and over will receive their statements in their MyChart account directly unless someone else is designated as their legal guardian. To gain access to an 18+ family members billing in MyChart, please contact the MyChart support team at 1-855-FAIRVIEW.
M Health Fairview patients with active MyChart accounts receive billing statements and estimates electronically via MyChart instead of receiving hard copies via U.S. mail.
Patients will receive a courtesy call as a reminder of outstanding balances.
If you had already declined paperless billing prior to this transition, you will not be automatically enrolled in paperless billing.
M Health Fairview contracts with outside groups to provide certain services––including but not limited to anesthesia, emergency room, and radiology. For those types of services, you will continue to receive paper statements unless you have made prior arrangements with those outside companies.
Our text-to-pay option is available for patients who have opted in to texts with a payment method saved in MyChart.
On 2/21/2024, Change Healthcare experienced a cyber attack. Due to this, we were unable to submit claims or receive insurance payments. This caused delays in billing patients for some services provided during that time.
There will be no financial impact to patients due to the delays and at this time, we have not been made aware of any breech of patient data.
Change Healthcare is the clearinghouse M Health Fairview uses to submit claims to insurance.
We were unable to submit claims or receive payments from some insurance companies between February and Mid-April due to the Change Healthcare cyber attack. Claims from that time are now being processed and statements for self-pay balances are starting to be sent out as of late April.
No, no late fees or additional charges will be added to your statements. You have 5 billing statements to either pay off your balances or set up payment arrangements without any financial penalty.
