When an illness or health problem occurs, your life is challenged in new and sometimes difficult ways. At Fairview's Acute Rehabilitation Center, our goal is to help you become as independent as possible so you can safely return to your home and community.
Our rehabilitation team includes experts with specialized training in caring for complex medical conditions. Our job is to help you return to daily activities. You and your family are the most important part of the care team. Together, we discuss you treatment goals and create a care plan that fits your needs and lifestyle.
The Firsts Steps to Recovery
Once you are settled in at Fairview Acute Rehabilitation Center, our team will meet and talk with you and your family about your hopes for returning to your daily routine, including work and leisure.
Your care plan focuses on your goals throughout your stay, such as re-learning how to climb stairs or eat.
You and your family are the most important part of our team. That’s why we regularly meet with you to talk about your progress and plan for your eventual transition home.
You can expect to complete three hours of therapy each day. Physical therapists, occupational therapists and speech-language pathologists help you to become more independent. Your family is encouraged to participate in your therapy sessions whenever possible. Rehabilitation nurses help you work toward your goals throughout the day.
We encourage you to be up and about as much as you can during the day. This will help you return to your normal activity level.
Specialized Care in Real-Life Settings
You may move from the hospital to the Fairview Acute Rehabilitation Center when your doctor and other medical specialists decide you’re ready for a higher level of activity and self-sufficiency.
Here you will work with doctors, nurses, therapists, dietitians, social workers and other experts every day. Much of your treatment takes place in a real-life setting, focusing on tasks that will assure your safety at home. You may practice getting in and out of a car, completing simple chores, shopping in a grocery store or cooking in a kitchen.
Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation was awarded to Acute Rehabilitation Services for hospital inpatient rehabilitation programs for adults, children and adolescents as well as stroke specialty programs for adults. Our facility completed the 2018 CARF survey with no recommendations. This accomplishment is only achieved on 3% of CARF surveys.Outcomes Reports
The Acute Rehabilitation Center offers comprehensive care of stroke patients to the complex rehabilitation of those recovering from a wide range of illnesses. Our mission is to support our patients in their return to the community as freely and independently as possible in the areas that are most important to each of them.
