We help people who live with chronic pain from a variety of causes and offer treatment for conditions such as: Back pain Neck pain Myofascial pain syndrome Neuropathies, including causalgia Chronic Regional Pain Syndrome (CRPS) Postherpetic neuralgia Cancer pain Pelvic pain Often patients require a treatment plan that involves a variety of services including procedures, medication management, physical therapy, and behavioral health. Under our care management program, these services are coordinated through our office to assure a collaborative approach as we work to reduce and relieve your pain and return you to an active life. You must be referred to the M Health Fairview Pain Center by your primary care doctor or a specialist. After an initial assessment and physical exam, we'll design a program specifically for you. Procedures are performed on an outpatient basis, allowing you to keep a regular daily routine.

Intake forms for new patients: Prior to your first appointment, please complete the First Appointment Intake Form and mail to the Pain Center using the instructions on the form.

For acupuncture patients: Prior to your acupuncture appointment, please complete the Acupuncture Intake Form and mail to the Pain Center using the instructions on the form.

For referring providers: If you are a referring provider, please complete the Pain Center Referral Form.
