Becoming a MyChart proxy lets caregivers (for example, a parent or spouse) access another patient’s health information and manage their care. Proxy access may be limited depending on the patient’s age and type of proxy relationship. Proxies are required to have their own MyChart account.

Proxy Access in the Clinic
- Parents can request full proxy access for minor patients age 0-11 years during any routine clinic visit, or by contacting MyChart Support at 855-513-5513 or MyChartHelp@fairview.org. When the patient turns 12 years old, proxy access automatically changes to limited access to align with Minnesota state law.
- Legal custodians, foster parents, stepparents, non-custodial parents, or persons with shared parental authority must complete the MyChart Proxy Request Form and provide legal documentation or approval of the custodial parent to validate their authority (if not already on file). Access must be validated every five years to ensure there have not been changes to the guardianship. Access for Emergency or Temporary Guardians will expire on the date indicated in the court order.

MyChart Proxy FAQ
Yes, once a patient is marked as deceased in their electronic medical record, all MyChart proxy access will immediately expire. If you need access to a deceased patient’s medical records outside of MyChart, submit an authorization form at www.fairview.org/medical-records.
Email the completed form to honoringchoices@fairview.org along with any legal documents to validate authority (if not already on file).
1. Go to the MyChart Menu and select the Sharing Hub (search for “proxy” or “sharing hub”).
2. Under Who do you want to share your health information with?, select A family member, close friend, or caretaker.
3. Select Manage ongoing access to your MyChart account.
4. Select + Invite someone.
5. Enter the name and email address of the proxy you wish to grant access to your MyChart, read the terms and conditions before selecting the checkbox. Once ready, select Send invite.
6. The prospective proxy will receive an email invitation to gain proxy access to your MyChart. The prospective proxy will log in to MyChart using their own credentials, and confirm they know your date of birth. After entering the correct date of birth and selecting Accept invitation, adult to adult proxy access is granted between you and your proxy.
Note: If needed, once the adult to adult proxy access is granted, either you or your proxy can go to Sharing Hub and Friends and Family Access activity and select Revoke to revoke the proxy access.
