Advance Care Planning and Health Care Directives
When it comes to decisions about your health, it’s important that your health care goals, values and wishes are known. In the event of a sudden injury or illness, you may not be able to communicate your choices. We encourage you to begin by thinking about what matters most to you. Have conversations with family, friends, cultural and/or faith leaders, and your health care team. Then, share your goals and wishes for current and future healthcare so your voice is heard when treatment decisions need to be made. See the resources listed below for help with talking about and sharing what is most important to you.

A Health Care Directive is a legal document where you describe your health care treatment goals, your values, and name the people (Health Care Agents) you trust to make decisions for you when you are unable.
We have several options for Health Care Directives to best meet your needs.

- PREPARE for your care: Online step-by-step tool to assist with completing a Health Care Directive (30 minutes to complete)
If you are a Fairview patient, you can submit your Health Care Directive several ways:
- Drop off a COPY at any of our clinical locations
- Email to: honoringchoices@fairview.org
- Upload in MyChart (Advance Care Planning section)
