All impacted patients received letter in the mail with information about our exit from this program on November 28, 2025. Please refer to that letter for further information. Please note that the obligation to preserve all ACO REACH records continues for ten (10) years following the termination of our participation in ACO REACH. Your organization is no longer permitted to market the Fairview Health Services ACO REACH program moving forward, and implementation of any Benefit Enhancements and Beneficiary Engagement Incentives are terminated effective December 31, 2025, including the 3-Day SNF Waiver.
Data sources and any derivative data pursuant to the HIPAA-Covered Data Disclosure Request Form that have been disclosed to Fairview Health Services and to you in order to carry out performance in the program may be retained until 30 days after completion of the final financial settlement of the final performance year in ACO REACH (Q3 of 2026). At that time such data will need to be destroyed, with the exception of data which has been incorporated into the subject beneficiaries’ medical records that are part of a designated record set under HIPAA. Any data destruction needs will be communicated more specifically closer to that time.
We have participated in the ACO REACH model since 2022 (previously the Global and Professional Direct Contracting Model) and appreciate your collaboration and support. Our participation has yielded strong results over the past few years and our learnings have positioned us well for participation in the Medicare Shared Savings Program (MSSP) beginning in 2026.
In accordance with the Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) Performance Year (PY) 2023, ACO REACH entities are required to publicly report certain information from the start of each PY.
Based on Section 14.01 of the PY 2023 Participation Agreement, each Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) entity shall report the following organizational information on a publicly accessible website maintained by the ACO REACH entity.
