Overview

Integrated Behavioral Health is the care that results from Primary Care Providers and Behavioral Health Clinician’s (BHC’s) partnering together with patients and families to provide a patient-centered, whole person approach to meet your medical and behavioral health needs.

Our approach

A Behavioral Health Clinician is a licensed mental health professional who can help when behaviors, stress, worries or emotional concerns about physical or other life problems are interfering with a person’s daily life and/or overall health. At M Health Fairview, integrated care is a team sport. The BHC and your Primary Care Provider work together to care for you by considering the physical, behavioral and emotional aspects of your health concerns. They partner with you to determine a care plan to support your whole health that will work best for you. We are here when you need us. Our passionate, dedicated and caring team is driven to help you discover real and lasting change.

Services may include: a comprehensive assessment, brief psychotherapy interventions, information, resources and referrals, and coordination and continuity of care with your primary care team.

Conditions treated include: Depression, anxiety, worry, panic, stress management, mood disorders, grief and loss, relationship and parenting concerns, coping with a new or chronic condition, substance use concerns.
