We offer onsite Primary Care to people at Assisted Living and Skilled Nursing Facilities (nursing homes) and onsite medical care for patients in Transitional Care Units, helping to bridge the transition from a hospital stay back to home. Our onsite medical team is a Nurse Practitioner or Physician Assistant and a Physician.

M Health Fairview Geriatrics Services works in partnership with Fairview Partners, a program that provides added benefits that can enhance a resident’s care experience. We offer this program in various locations through the M Health Fairview service network.
Our approach
Our providers work together with nursing facility care teams to provide primary care for patients living in Assisted Living and Skilled Nursing Facilities (nursing homes). We also provide care for patients in Transitional Care Units that are transitioning from hospital to home. Nurse Practitioners or Physician Assistants facilitate day to day patient care needs in partnership with Physicians to help patients return to their homes and primary providers in the community.
