Trained service animals recognized under the Americans with Disabilities Act (ADA) are allowed at M Health Fairview facilities. M Health Fairview staff may ask patients screening questions upon arrival to assess whether an animal meets the definition of an ADA service animal. Service animals are expected to be clean, healthy, and controlled by their owners during their visit. Family pets and emotional support animals are not allowed at M Health Fairview facilities unless approved in advance by the patient's care team and infection prevention personnel. Please contact your care team with questions.

A Health Care Directive is a legal document where you describe your health care treatment goals, your values, and name the people (Health Care Agents) you trust to make decisions for you when you are unable.
We have several options for Health Care Directives to best meet your needs.
These tools give you a starting place as you begin to think about what matters most to you. You can use them for yourself, or to guide discussions with people you trust.
- Advance Care Planning Guide: Information about how to get started and how to complete a Health Care Directive.
- Advance Care Planning Terms to Know: Explanation of common terms in Advance Care Planning.
- Artificial Hydration and Nutrition: Information on treatments which provide fluid and food when you have difficulty swallowing or are too sick to eat on your own.
- Cardiopulmonary Resuscitation (CPR): Information about cardiopulmonary resuscitation (CPR), an emergency procedure used to try to restart a person’s heart beat or breathing.
- Dialysis: Information about the process that cleans waste from your blood when your kidneys can no longer do the job.
